My Clinical Outcomes is thrilled to share the Fix IT in Healthcare Prize alongside Dr Ollie Minton, Rob Herriott and their incredible colleagues at University Hospitals Sussex NHS Foundation Trust (UHS) to implement and scale Patient Reported Outcomes Measures (PROMs) in routine NHS Cancer Care.

The Fix IT in Healthcare Prize rewards excellent innovation in digital healthcare, particularly digital initiatives that have, or promise to, improve patient safety and staff well-being within the NHS or healthcare more widely.

The project between MCO and NHS Sussex has received huge support over the past few years from NHS England, Sussex Cancer Fund, Unity Insights, My University Hospitals Sussex, BMJ and many more groups. It is now being sustained and grown through ongoing collaboration with NHS Sussex ICB, which is scaling the routine and integrated use of PROMs to other clinical areas beyond oncology and enhanced supportive care.

If this new technology-enabled service were to be implemented nationally, it would be transformational for cancer patients and potentially save the NHS millions.

Using PROMs and My Clinical Outcomes in Cancer Care - Key Note
at Intelligent Health UK - 2023 Summit

9th June 2023

At the Intelligent Health UK 2023 summit, in his keynote speech ‘ When personalised care meets technology’ , Prof. Richard Simcock - Chief Medical Officer at Macmillan Cancer Support and one of UK’s leading breast oncologists - championed the use of PROMs and My Clinical Outcomes (MCO) in cancer care.

He also shared his ideas on how PROMs and AI can be combined to achieve great benefits in one of the current biggest challenges in healthcare: saving clinical time.

Prof. Simcock’s shared the benefits of using MCO with his patients:

  • MCO allows patients to regularly report on their quality of life so that ‘ we can triage problems efficiently in a good quality conversation (with patients)’, and ‘it makes clinic efficient’ .

  • MCO also allows ‘at scale, and quickly, to see what are the problems for the community of patients that I treat’ and to implement improvement initiatives.

NHS England Begins Rollout of Mandatory Outcomes Registry

18th May 2023

NHS England will launch the new national mandatory medical device outcome registry this month in response to Baroness Cumberlege’s “First Do No Harm” review.  The registry, called the Medical Device Outcome Registry (MDOR), aims to improve patient safety and outcomes by gathering data on high-risk procedures, clinicians, and device usage.

As well as key features such as native barcode scanning of the unique device identifier, MCO is pleased to see that MDOR will support the use of digital patient-reported outcome measures (PROMs) to enable feedback on the outcomes of a procedure to be collected directly from patients themselves.

A phased roll-out of MDOR is planned in selected hospitals and is due to be complete by spring 2024.

Laing Buisson’s Private Healthcare Summit - 2023

17th May 2023

Value Based Healthcare Session Panel

We thoroughly enjoyed going back to Laing Buisson’s Private Healthcare Summit - 2023.

This year, the Conference featured many of the UK's largest operators, insurers, and interested parties. Amongst other senior delegates at the conference, several of MCO’s clients and collaborators like The London Clinic and PHIN (Private Healthcare Information Network),

Our highlights from the day:

  • It was great to see the emphasis on value-based healthcare (VBHC) on the agenda and to register consensus across the VBHC session’s panel. VBHC is not only both inevitable and desirable, but that technology is a key enabler to achieving this.

  • Updates from PHIN including that collection and use of PROMs in UK private sector is now encouraged for all eligible procedures regardless of any minimum volume thresholds. See here for more details.

This emphasis aims to facilitate improvement efforts and ensure accountability within the healthcare system.

The recommendations on collecting PROMs assessments on a regular basis can be found on page 63 of the Hewitt Review.

PROMs Implementation Status included in “World’s Best Hospitals 2023”

16th March 2023

Newsweek and global data firm Statista have published their fifth annual listing of the World's Best Hospitals 2023. This year, they have ranked over 2,300 hospitals in 28 countries - listing top 250 global hospitals, as well as the best hospitals by country including a listing of top specialty hospitals.

For the first time this year, hospitals were surveyed regarding their PROMs implementation status. The addition of PROMs implementation status as a new pillar in the scoring model highlights the importance of patient perception of their functional well-being and quality of life in determining hospital quality metrics.

It is fantastic to see the number of UK NHS and private hospitals on this list, some of which My Clinical Outcomes work with directly.

Remote Symptom Reporting Systems for Patients May Reduce Outpatient Waiting Lists

9th February 2023

A new Review article published in the Journal of the Royal Society of Medicine highlights the use of electronic patient reported outcomes (ePRO) systems for triaging clinical care. Drawing on international examples, the authors summarise the benefits and challenges of implementing an ePRO system including:

  • Reduction in outpatient appointments

  • Better understanding and documentation of patients’ symptoms

  • Initiation of timely interventions

  • Facilitation of patient–clinician interactions/communication

The utilisation of ePROs in the delivery of outpatient care could potentially allow a more responsive healthcare system, reduce demand for clinic appointments, reduce time to care with associated improved outcomes and enhance cost-effectiveness of healthcare delivery – all of which are beneficial for patients, their families and society.

UHS Health Economic Analysis Published

4th April 2022

Unity Insights, formerly Kent, Surrey and Sussex Academic Health Sciences Network (AHSN), undertook health economic modelling alongside quantitative and qualitative data collection to provide a report evaluating health economic outcomes of the supportive oncology project.

“(Enhanced Supportive Care) provides earlier access to supportive care for patients who are towards end-of-life or with treatable but not curable cancer. It is typical to see substantial use of the healthcare system within these cohorts; therefore, the primary benefits identified for this programme focus on a reduction in the use of hospital resources to generate healthcare cost savings.”

“The service is not limited to patients with palliative cancer or within the Sussex region - other patients with chronic conditions would likely benefit from ESC.”

Key findings included:

🚑 Reduction in unplanned admissions & length of stay

🤝 Proactive & earlier supportive care

💰 c.£1.20 back for every £1 spent

📈 c.£11.2M saved over 5yrs if scaled across SE #NHS region.

MCO Cancer Innovation Challenge Project Showcased at Scottish Parliament, January 2020

Our collaboration with NHS Ayrshire and Arran was celebrated at ‘The Data Lab: Supporting Transformative Projects’ reception in the Scottish Parliament building. As well as speeches from Clare Adamson MSP and Kate Forbes MSP, the keynote talk of the evening was by Dr Peter MacLean, oncologist and Director of Cancer Services at NHS Ayrshire & Arran, who discussed our work over the past few years and the real difference MCO has made to the efficiency of his clinics and the impact it has had on his patients.

This partnership was our first Cancer Innovation Challenge project and is now in its third year of improving outcomes for patients. Led by The Data Lab and funded through the Scottish Funding Council, the Challenge aims to transform the lives of cancer patients through the innovative use of data, helping Scotland become a world leading carer for people with cancer.

“My Clinical Outcomes started working with NHS Ayrshire and Arran and are now working across 5 health boards…so who says we can’t scale innovation in Scotland?!”

PHIN Publish Health Outcomes Data,
December 2019

Information concerning health outcomes for patients receiving privately-funded care in the UK has been published for the first time. PHIN’s publication covers over 100 independent hospitals and NHS Private Patient Units - a major step in helping to inform patient choices.

“Patients should have access to this information so that they can make informed choices about their care and treatment and we welcome the increased transparency that this data provides.”

Professor Ted Baker, Care Quality Commission Chief Inspector of Hospitals, 2019

MCO are pleased to be successfully supporting our clients with this important initiative by facilitating the digital, remote collection of PROMs and managing data submission to PHIN. We look forward to contributing to further publications and assisting our clients in analysing their performance against industry benchmarks.

PROMs not only provide reassurance about the quality of services, but also help specialists to confirm what matters to their patients before treatment, and measure how treatment has met those needs following treatment."

'For some cancer patients, monitoring symptoms can extend their lives', November 2019

The use of Patient-Reported Outcome Measures to improve cancer survival was highlighted again in this Washington Post article.

Building upon key studies by Basch et al., the article discusses research by Cancer Care Ontario between 2007 and 2015 whereby patients who answered at least one survey during treatment were more than half as likely to have died during the study period than those who did not answer any surveys.

The article gives further backing to the use of patient reported outcomes in routine clinical practice and emphasises the importance of the shift towards more patient-centred care across oncology.

“There is a strong argument to be made for using these measures in routine care…this idea, that we have to change how we do our assessments with patients, is paradigm-shifting.”

Dr Lisa Barbera, University of Calgary, 2019

Trigger Project Presented at NCRI Conference, November 2019

As our partnership with Macmillan Cancer Support and The Royal College of Radiologists continues and expands to other NHS sites, Project Trigger was presented at the National Cancer Research Institute (NCRI) Conference in Glasgow.

Our partners discussed the recent successes of the project in supporting people “living with and beyond cancer”, with 336 patients having registered in the first 9 months, tracking symptoms after the completion of pelvic radiotherapy in order that they might be directed to appropriate services if their online assessments raise suggest potential late toxicity. 85% of patients reported that they find it a helpful addition.

“These promising results show that electronic PROMS can be introduced in radiotherapy departments using a low resource model….Patients engage with electronic PROMs projects, and find them useful.”

Trigger Project Featured In Journal of Clinical Oncology, August 2019

MCO recently featured in the Journal of Clinical Oncology, in relation to our partnership with Macmillan Cancer Support and The Royal College of Radiologists.

In ‘The Challenge of Introducing Electronic Patient-Reported Outcome Measures Into a Radiotherapy Service’, our partners discuss how they have been using the platform to support radical pelvic radiotherapy patients across multiple sites, collecting data without the influence of physician bias.

“This could significantly improve the experience of patients undergoing treatment in the future, as in light of ongoing resource and workforce constraints, electronic collection of these data could provide stratified follow-up of patients with cancer, detecting those with toxicities and supporting remotely those who are not currently experiencing side-effects.”

PROMs at the American Society of Clinical Oncology conference, May 2018

The benefits of incorporating digital patient-reported outcomes measurement into cancer treatment featured prominently again at this year’s American Society of Clinical Oncology (ASCO) conference.

The beneficial results from last year’s publication in JAMA by Basch et al. were recapped, specifically that in the study group using digital PROMs alongside usual treatments:

  • 31% reported quality of life benefits at 6 months

  • there were 7% fewer Emergency Room visits

  • mean overall survival was 5 months longer (translating to an absolute 5 year survival benefit of 8%)

There was also a discussion about barriers to adoption of PROs in routine practice that centred on the need for cost-effective, specialist digital tools.

Developments in PROMs for Mental Health patients, January 2018

In a new paper in the British Journal of Psychiatry, Keetharuth and colleagues have tested the benefits of using the Recovering Quality of Life (ReQOL) PROM, for the first time aiming to address the challenge of systematically capturing a succinct, valid and reliable measure but that is broad enough to be used across services and patient groups and goes further than just facilitating diagnosis. The measure is determined to properly capture and differentiate patients and has generated enthusiasm with the NHS England National Medical Director commenting as a result, “Is this [2018] the year the mental health world cracks the data barrier? - routine PROMs, PREMs [are] the only sustainable way to make progress and gain support.”

A discussion of the paper on the respected “The Mental Elf” blog notes that "PROMs have been promoted for some time, but ...challenge of widespread implementation across a larger healthcare organisation and system, with ‘real-time’ digital usage and multi-level feedback and analysis for service users, clinicians, teams, and services – a problem as yet generally not cracked.”

MCO are working to leverage our technology to help address this significant need in an important disease area.

Forbes identify PROMs as one of "7 Key Cancer Trends for 2018”, January 2018

To mark the New Year, Forbes Magazine has published a list of major trends that they expect to affect the lives of patients living with cancer in 2018. At number 6 is the widespread adoption of patient-reported outcomes: “What patients feel matters… As patients and doctors weight treatments that might improve quality-of-life, without necessarily improving survival, then PROs become more relevant."

We agree but would go further to cite the research from 2017 - see here - that also demonstrated that the use of digital patient-reported outcome measurement could help hospitals and doctors manage patients better and actually improve survival by up to 5 months.

Research: Outcomes Data Demonstrates That Common Shoulder Surgery is Ineffective,  November 2017

A UK randomised controlled trial involving 313 patients, 51 surgeons and 32 hospitals used post-treatment Patient Reported Outcome Measures (PROMs) data collected at six and twelve months to demonstrate that a common surgical treatment of shoulder pain (arthroscopic sub-acromial decompression) is no more effective than not operating on these patients.

This surgery is commonly performed in the UK and this study shows that simply by measuring outcomes data provides essential new information about the effectiveness of treatments. Routinely collected outcomes data has the opportunity to improve care for all patients while reducing the significant costs associated with providing ineffective treatments.

Research: Remote PROMs Improve Survival in Cancer Patients, June 2017

A team from Memorial Sloan Kettering Cancer Centre presented a randomised controlled trial that showed a median increase of 5 months survival for patients with advanced cancer who were asked to remotely track their outcomes using a digital platform like 'My Clinical Outcomes'. By comparison, only one of the seven new and very expensive drugs approved for use for advanced cancer patients in the US in 2016 had a greater impact on survival. Potential mechanisms included enabling earlier responsiveness to patient symptoms preventing later adverse consequences and patients being able to tolerate chemotherapy longer than usual. The implication is clear, however, that remote outcomes monitoring should now be a routine part of high quality cancer care.

Health Datapalooza, June 2014

Health Datapalooza is a US national health information technology conference, help annually in Washington DC, which focuses on, "liberating health data, and bringing together the companies, startups, academics, government agencies, and individuals with the newest and most innovative and effective uses of heath data to improve patient outcomes". Tim was invited by US Ministry of Health's National Coordinator of Health IT to take part in a panel discussion in the General Track on the Tuesday, co-hosted with NHS England, on "Accelerating Widespread Collection and Use of Patient Reported Outcomes in Electronic Technology in the United States & United Kingdom" and took part via a live weblink.

The panel brought together US patient reported outcomes systems suppliers with academics and clinical registry experts interested in furthering UK and US collaboration in the area. As well as being an excellent opportunity to present and discuss our experiences in the UK, the session demonstrated that many of the challenges of implementation, engaging patients, making systems clinical useful and useable, as well as how data should best be analysed, insight communicated and advancements in practices made as a result were common to all and it therefore makes great sense to work together to share understanding.

Health 2.0, April 2014

Part of the international Health 2.0 movement, this London community is for anyone - technology enthusiasts, developers, students, physicians, nurses, pharmacists, patients, designers, social workers and doctors - interested in innovation in the delivery of healthcare and health related technology.

Tim presented at the session asking the question, "Does giving patients access to their own data actually increase engagement and improve outcomes?". Other speakers at the event were Fiona McKenzie, patient and Patient Insight & Involvement Lead at UCL Partners, Sam Meikle, Director at London Connect and Alex Tarling, a usability and user experience consultant. Time started by acknowledging that we are still in the very early days of being able to answer this question, but that being more transparent with patients about their own health data as actually measuring the clinical outcomes that matter to them resulting from all aspects of care were essential first steps.
